How to register

Check you live within the boundary area

To access our registration process, first check you are in our catchment area. If you have any questions about your eligibilty join this surgery please contact us.

Our practice area covers all of Woodway Park and Potters Green. We also cover some parts of Wyken and Walsgrave, you would need to check to see whether we cover your address.

If you move out of the area please complete a change of contact details form

Temporary services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Accountable GP

When you are registered with our surgery, you will be be appointed an accountable GP, also known as your Usual GP. This is the GP that will appear on your medical records. It is assigned at random and you do not need to see that GP when you want an appointment. If you would like to change your accountable GP, please speak to reception and we will change it on request.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 9am to 5pm or complete our secure online form

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS app or website.

The NHS website has information about using linked profiles to access services for someone else.
