Sick Certificates

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.

If you require a note for the first seven days, this will attract a private sick note fee.

To request a sick note

You can request a new sick note or to renew a certificate in the following ways:

  • Use an online form to request an appointment. We will respond within 3 working days. We are open Monday to Friday, 8am to 6.30pm.
  • Phone us Monday to Friday, 8am to 6.30pm.
